2. Fill out your section (and find a friend to do the same!).

3. Then take it to your local blood donating center.

4. Once you’ve donated, please ask the nurse who helped you to write their initials on the form, as well as the amount donated.

5. Finally, email, fax or mail your form, using the information below.

That’s it! Once we received your form, we will add it to the total of the blood given thus far and put your first name and last initial on the featured story page.
